Information we collect
When a practice submits a listing request, we collect the practice and contact details entered in the form, including the submitter's name, work email, optional phone number, website and service information.
Do not submit client names, health records, medical histories or other sensitive patient information. Allied Match does not need that information.
How we use it
- Review and verify a proposed provider profile.
- Contact the practice about corrections and periodic availability updates.
- Protect the directory from inaccurate or misleading listings.
Publication
Submitting information does not publish it automatically. We review submissions first. Personal contact details intended only for verification are not displayed unless the submitter clearly asks us to use them as public practice contact details.
Retention and requests
We keep submission records while they are needed to operate and validate the pilot. A practice may request access, correction or deletion through the listing correction process. We do not sell submitted personal information.
